    Are you sure in RMPrepUSB's GUI you had the partition size field in 1 set to "MAX" rather than that specific size in MB, also in field 4 selected radio button for FAT32 not FAT16. What does Windows Disk Management now show as its file format when you connect the Approach in Mass Storage Mode, also does it show a 14.7MB partition followed by unallocated space for example?

    Yes, I did set to MAX but choose FAT16 (not FAT32)...That might be the reason for the smaller size.
    PartitionWizard is showing an Unallocated partition

    I tried to merge the two partition but PartitionWizard merges after converting to NTFS.
    After merging using NTFS, PartitionWizard Free version does not give you the opportunity to revert to FAT and the watch would not see the file again.

    So I reformat again to FAT using Windows Disk Management...And this time SUCCESS!
    After the Master Reset (Off, Down key + ON, erase user data), the Garmin folder and the xml file were recreated.

    I cannot explain why it worked this time, but it did.
    I have now 29MB of available space, load the 3 regions and confirmed that everything is back to normal.
