I've been using a branding.zip from a Brazilian website for a while and it's the closest one I can find that contains the vast majority of USA-related facilities. The only drawback is that there are a lot of missing icons for the facilities, but the ones they do have are perfect! I was thinking maybe you or Varso could look inside the branding.zip and maybe pick up a few hints to avoid the problems you are having. I have uploaded a copy of the branding.zip and correlating poi_visibility.txt I am using for *Nextgen* version.
