Updater.exe work on XP fine, I don't use Windows 7. But if you don't install MapSource, you need Garmin USB drivers.

I never had problems with Updater in my Win 7, I could flash 1200 and 3790 to asian firmwares and return back, np

Install usb drivers [Only registered and activated users can see links. Click Here To Register...] and try again or try diferent cable to connect nuvi to pc.
When you open updater.exe click on USB and then click OK
If you have win 7 try run as administrator or change to windows xp sp3 compatibility.

I did run it without problems on Win7 x64:
Set the properties to be "XP compatibility mode" and "run as administrator" for that.
Used the "press 5 sec on battery" method - the device didn't show in windows but it "dinged" so I run the updater anyway.
Now... I have a 1390T with 5.40. Update sucessfull. Put back the GUPDATE.GCD.
And... I am confused, what is new?
Garmin says that it already have "Lane assist with junction view" and I had seen lane assist and speed limits in action. Didn't see a junction view maybe because I didn't get in the right part of country.
I guess for me was useless this update :)
I was hopping to fix at least this problem: [Only registered and activated users can see links. Click Here To Register...]

You more than likely got saddled with a 51MB Junction View file, especially if you updated to 2012.20 recently. If you did do that update, there is a 752MB file available on the CNNANT 2012.20 thread in the Maps sub-forum which will cure all your Junction View woes.
For your 1390T, you might want to check to make sure you still have bluetooth capability, since if I'm not mistaken the 1390 has bluetooth.
