When you edit a POI in EPE, you have the possibility of selecting a predefined symbols for your POI. I know it does not always display correctly on Win 7 (I have to fix this), but this feature really works when adding a symbol in your Favorites. POI Loader ignores it when making custom POI. Since I have all the bitmaps, I could automatically add the symbols when creating the GPI. Here's what I propose: if no symbol are set in the POI itself, I will use the one from the filename.bmp. If a symbol is set, EPE will add the symbol in the .gpi. If you do not want this, just use the global "Replace Fields" function to replace the Symbol field by "" (nothing).
I will probably not do this right away, but I will look at it. Maybe it is not possible to add one symbol per POI. It would an easy way to add symbols without having to find bitmaps for them. Also, it allows you to have different symbols inside a single POI file.

Yes, you are missing some libraries. Just read EPE's troubleshoot section here:
Use the installer. This will install the needed libraries. Then overwrite with the latest version.Code:http://turboccc.wikispaces.com/Extra_POI_Editor#Extra_POI_Editor-Troubleshooting
I checked a bit more about the predefined symbols. I will be able to add them, but it needs to be in a category. I already have "Export by Category" (which you may test already with .gpi files). I will add "Export by Symbols". All POI with the same symbols will be regrouped together with that symbol on the map. The first level of category will be the symbol name.
For example:
Parking
- Poi 1
- Poi 2
- ...
Rest Areas
- Poi 1
- Poi 2
- ...
etc...
I think it will be a good start.
I also worked on adding WAV files (with the same name as the poi file, but with a .wav extension). WAV needs to be converted to 44100 Hz, so I support only 44100 Hz audio wav for now. It is done, but I need to test it first. The next step is to use sox.exe to make conversions from 22050 Hz (or any other sampling rates) to 44100 Hz. I do no think that MP3 will be a problem, but I need to test that as well. Iti will work only on MP3-capable GPS.
Here's EPE v5.07 beta. In this release, I added speed and proximity alert. I am not sure what will be the alert without .wav file loading, but I guess there is a default sound. I disabled .wav loading for now. It was working, but I want to integrate sox.exe to convert unsupported audio file format. Probably in a next release. I also added Export by Symbol. It does not use yet the symbol as the map icon, but this will be coming too.

I tested but the the file doesn't show up in my Nuvi 765.
Here is what I did.
I used a gpx file which loads fine in EPE 5.07. Contains the Metro of Taipei called MRT.gpx with different exits.
I made also a bmp file called MRT.bmp and placed it in the same folder.
Export by Category, saved as MRT.gpi (Garmin gpi), copied the file to the POI folder on my Nuvi and nothing showed up.
Attached the MRT.bmp, the MRT.gpx file and the generated MRT.gpi.
Maybe you can see what is going wrong

Just use Paint to modified .bmp to 256 colors. (24 bits not work :) )
