The additional protection is built into the garmin-nav file in the garmin-nav.app folder. You could try replacing the garmin-nav file from the 2.2 version in the 2.3 installation. Check if ownership and permission is set correctly. If it works with the additional features enabled (like glympse), could you please upload the 2.3 ipa after removing the maps, jcv and sid?

Does anyone know how to access or add the in-app purchased content "Video Display Integration"?

I already tried that (replace 2.3 garmin-nav file with 2.2 version)--the app will start up but will crash once it gets to the main screen; besides, even if it could work, it probably wouldn't have the stuff about Glympse, iCloud, etc. compiled into its 2.2 binary executable file.

I suspect it's looking for features characteristic of a jailbroken phone (e.g., existence of Cydia, SSH, etc.)...

    Update--good news and bad news...

Good news: I was able to get Garmin 2.3 to work on a jailbroken phone--it does apparently indeed check the /etc/hosts file for any entries that contain the string "garmin"...but it's not (yet) smart enough to check if a third-party firewall app (e.g., Firewall IP) is installed. IOW--to get it to work, you simply get rid of any "garmin" entries in /etc/hosts, install Firewall IP and tell it to block gold.garmin.com...voila--working 2.3 on JB iPhone.

Bad news: Apparently Garmin wised up and now starting with version 2.3 it does all of its server-side checks exclusively through gold.garmin.com--so this means no Extras in-app access, no Google Local Search, no Fuel Prices, no Weather, no traffic hacks...no more needing to allow access to downloadg/silver/bronze/triplew because Garmin did away with that with 2.3 and up. They can't (yet) completely block those hostnames snce version 2.2 and older are compiled to still do their in-app store access checks through those hostnames.

Oh well...******* 2.3 is useless on an iPhone since pretty much none of the new features introduced in 2.3 will work anyways since they need to go through gold.garmin.com and you can't unblock it without getting the authentication error...so you either stick with 2.2 or buy a legit 2.4 (since 2.3 no longer available in the iTunes App Store).
